Vein Stripping and Ligation
If your are not a candidate for EVLT you still have options
Vein stripping and ligation is an old operation that has been the only option for the management of significant varicose vein caused by trunkal venous insufficiency. Trunkal veins are large, high pressure veins that are the root cause of varicose veins in many patients.
Stripping requires a general or spinal anesthetic and about two hours of surgery. The greater saphenous or lesser saphenous vein is stripped (pulled... see video) out of the leg using a special tool. The surgery always leaves at least three incisions on your skin. One in the groin or behind the knee, one on the calf or ankle and several over varicose branches on your calf or thigh.
Because of the incisions and discomfort post-op, seven to fourteen days off work are usually needed to recover.

Despite a higher complication rate, more pain and downtime there are still certain patients who should undergo stripping instead of EVLT. These decisions are based on the characteristics of the vein to be removed or lasered. This decision is made by your surgeon during the duplex ultrasound examination. Most greater and lesser saphenous veins are amenable to EVLT making stripping somewhat of an obsolete operation.

Hematoma (blood clot in leg) -common
Lower leg numbness-10%
Failure- 15%
Incisions 2-6+
DVT- 0.1- 0.5%
